DEADLINE FOR REGISTRATION IS MONDAY, JANUARY 23, 2012

Two shifts: 11am and 2pm

Please arrive 30 minutes early for check-in.

All registrations are due by Monday, January 23, 2012. You can download a registration form or additional pledge forms here.

Please make checks payable to Children’s Miracle Network.

Join us! Participating is easy and fun.

1. Get your team together. There is a maximum of five bowlers per team.
2. Call the Faxton St. Luke’s Healthcare Foundation by Monday, January 23, 2012, at (315) 624-5600 to register over the phone. Or, you can download a registration form at faxtonstlukes.com.
3. Collect donations to meet or exceed your minimum pledge. The more money you raise, the more chances you have at winning some really cool prizes and making a big difference to local kids. PLUS…all the money raised helps sick children get better.
4. Need more forms or information about the event? Visit us online at faxtonstlukes.com or call (315) 624-5600.
5. Arrive at least 30 minutes prior to the start of your bowling shift on Sunday.
6. Our friends at AMF Pin-O-Rama have great food and drinks available for you to purchase. Please do not bring in outside food or beverages.

Pledge Information

Minimum Pledge for Children (16 and under) - $25
(bumpers are available)

Minimum Pledge for Adults - $40

Incentives for Paid Pledges

All participants receive a gift with their minimum paid pledge of $25 for children (16 and under) and $40 for adults, along with three games of bowling and shoe rental.

Plus:

• Minimum paid individual pledge: $60 to $119

  A teddy bear will be given to a child receiving care at Faxton St. Luke’s Healthcare on your behalf and you will receive a CMN Hospitals tote bag.

• Minimum paid individual pledge: $120 to $199

  Two teddy bears will be given to children receiving care at Faxton St. Luke’s Healthcare on your behalf and you will receive a CMN Hospitals t-shirt.

• Minimum paid individual pledge: of $200+

  Three teddy bears will be donated to children receiving care at Faxton St. Luke’s Healthcare on your behalf and you will receive a CMN Hospitals sweatshirt and pin.

Additional prizes will be awarded to the top overall child fundraiser (16 and under) as well as the top overall adult fundraiser.

All proceeds benefit our Children’s Miracle Network Hospital at Faxton St. Luke’s Healthcare Foundation
