Adirondack Community Physicians
Adirondack Community Physicians has launched myACP patient portal, a secure website where patients can conveniently access their personal health information.
The patient portal assists you in better managing your health by allowing you to connect with your care providers, manage upcoming appointments, refill medications, check test results as they become available, pay your bill, request a referral, update your personal information and more!
All portal messages are encrypted to ensure security and only people you have authorized and your care team can view your information.
Patients receive a Personal Identification Number (PIN) at their office visit and instructions on how to register.
Call (315) 624-4ACP (4227) if you would like your patient portal PIN to be mailed to you.
About Adirondack Community Physicians
To meet the healthcare needs of residents in Oneida and Herkimer Counties, Faxton St. Luke's Healthcare has eight Adirondack Community Physicians medical offices located throughout the Mohawk Valley.
Our network of more than 40 physicians, nurse practitioners and physician assistants provides excellent care to those in rural and suburban areas. Our practitioners provide physicals, general exams, health education, immunizations and more. They coordinate diagnostic testing and refer you to specialists when necessary.
Patient satisfaction survey results consistently place this group of highly skilled physicians, nurses and support staff in the 95-98 percentile when compared to similar healthcare institutions throughout the country. Our ACP medical offices strive to build a culture of patient satisfaction, making it a part of who they are and not something "extra" that they do.
Faxton St. Luke's Healthcare's Adirondack Community Physicians offers you and your family convenience and better access to quality medical care. It’s care for you, close to you.
Our approach to patient care has earned our ACP medical offices a national designation as a Patient-Centered Medical Home. As the only primary care physician offices in our local area (between Albany and Syracuse) to have this recognition, learn more about how being a Patient-Centered Medical Home benefits our patients here.
National Committee for Quality Assurance Patient-Centered Medical Home Recognition
